The Care Team is comprised of the Care Navigator and the Care Support Committee, working together closely. The Care Team is available to discuss concerns related to aging in your home and end of life issues. Meanwhile, the Committee is available during a complex health situation to assess needs, help coordinate VV services, and offer relevant community service referrals. Generally, the Care Team visits members in their home. 

Examples of times when the Care Team might be helpful: dealing with a health setback, preparing for hospitalization and recuperation, dealing with short-term illnesses, confronting mortality, and/or need for help in accessing both VV and community services.

The Care Navigator reaches out to members to arrange a home visit before problems arise so as to get to know members, including their wishes, their challenges and their questions.

This is a program that pairs Valley Village members who make brief daily contacts with each other in order to facilitate help in the event of an emergency. It is a program especially helpful for single-person households or in situations where a spouse is unable to seek help in an emergency. Once you submit this form, you will be contacted within a week regarding a daily check-in partner.

Just One Call provides occasional assistance to Valley Village members who are not able to manage certain tasks well on their own. Volunteers provide transportation or home-based services to members.

Please provide at least 48 hours notice to schedule a volunteer service.

Paid vendor referral may be suggested when appropriate.

Companion Care is a service aimed at providing regular, personal connection and gentle activity with those who 1) can no longer drive and spend a lot of time alone OR 2) have mild dementia and need supervision while their caregiver gets a bit of respite. Training is provided for the respite care volunteers. The time commitment is 1-2 hours weekly.
